Key Responsibilities
  • Develop and maintain relationships with residents and their families, providing a consistent and caring approach.
  • Assess, observe, and evaluate residents' clinical conditions, documenting findings in their electronic medical records.
  • Update or modify care plans as needed to reflect changes in residents' care and service needs.
  • Administer medications accurately and timely, according to orders.
  • Participate in meetings related to resident outcomes and care.
  • Respond to residents, family, or responsible parties in a timely and professional manner.
  • Participate in the review of clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.
Requirements
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • Passion for caring for seniors and being a strong resident advocate.
  • Experience with electronic medical records is preferred.
