Patient Care Coordinator RN

We are seeking a compassionate and skilled Patient Care Coordinator RN to join our team at Virginia Mason Franciscan Health. As a Patient Care Coordinator RN, you will play a vital role in providing end-of-life care to patients and their families in a wide variety of settings, including homes, Adult Family Homes, Nursing Homes, Hospice House, and Assisted Livings.
Responsibilities:
- Coordinate and direct the interdisciplinary team, patient, family, and facility staff to ensure patient-centered care.
- Assess patient and family physical, psychosocial, cultural, and spiritual needs.
- Provide end-of-life education to patients, families, and facility staff.
- Advocate for patients and families to ensure patient-focused care.
Qualifications:
- Graduation from an accredited school of nursing, BSN preferred.
- Two years of related nursing work experience in an acute clinical care setting.
- Work experience in one of the following clinical specialty areas: Oncology, Med/Surg, Nutrition Support, Pediatrics, IV Therapy, Gerontology, and Home Health.
- Current licensure as an RN by the Washington State Board of Nursing.
- Current Healthcare Provider BLS Certification.
- Current Department of Health Food Handler's permit issued by the State of Washington.
Pay Range: $66.87/hour
