We are seeking a Part-time Registered Nurse Case Manager based out of our Keene office.
Role Responsibilities- Manage a defined caseload of patients and provide direct patient care and comprehensive nursing services to individuals in the community.
- Valid nursing license in the state of New Hampshire.
- Two or more years of experience in skilled care preferred.
- One or more year of experience in nursing supervision preferred.
- Recent wound care experience is beneficial.
- Current CPR certification.
- Reliable transportation.
