Long-Term Care Nurse MDS Coordinator Position

At Applewood Center - NH, we are seeking a highly skilled Registered Nurse to fill the role of MDS Coordinator. This position plays a critical part in ensuring high-quality care for our patients through assessment and care planning processes.
Salary: $45-$50 per hour
Job Description:
In this role, you will work collaboratively with an interdisciplinary team to complete the Minimum Data Set (MDS) and associated Care Area Assessments (CAAs). Your focus will be on patient needs and outcomes, contributing to improved well-being while providing ongoing education and support to nursing staff.
The successful candidate will have experience in a hospital or long-term care facility and knowledge of general, rehabilitative, and restorative nursing practices. They will also be familiar with current regulations and guidelines governing long-term care.
Responsibilities:
- Participate in the Patient Assessment Process through completion of items on the MDS and accompanying CAAs
- Assist in developing individualized patient care plans, identifying patient needs and projected outcomes as required by Federal and State regulations
- Contribute to the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, following RAI guidelines for completion, computer encoding, and electronic transmission of the MDS
- Identify changes in patient condition in conjunction with the interdisciplinary team and coordinate scheduling and completion of indicated assessments
- Assist with Activities of Daily Living training for new hires and through ongoing continuing education to existing staff
- Attend educational programs to maintain knowledge with current RAI/Prospective Payment System regulations
Qualifications:
- Must be a Registered Nurse currently licensed by the state and in good standing
- Previous experience in a hospital, long-term facility, or other healthcare-related facility is required
- Prior experience completing MDSs and Care Plans is required
- Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations, and guidelines governing long-term care
