Job Summary:

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at Granite Peak Home Health. As an LVN, you will play a vital role in providing high-quality patient care and support to our clients in the comfort of their own homes.

Key Responsibilities:

  • Develop and implement individualized care plans for patients, taking into account their unique needs and goals.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for enhancement.
  • Document patient progress and outcomes, maintaining accurate and detailed records.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
  • Work collaboratively with other healthcare professionals to ensure comprehensive care and support for patients and their families.

Requirements:

  • Current licensure as a Licensed Vocational Nurse (LVN) in the state of practice.
  • Minimum of one year of experience in home health care or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
