Nursing Care Specialist

We are seeking a highly skilled licensed practical nurse (LPN) to join our team at VNA & Hospice of the Southwest Region Inc. This is an excellent opportunity for a compassionate and dedicated professional to provide high-quality care to patients in their homes.
About the Job:
- This full-time LPN position offers a competitive salary, with an estimated annual wage of $65,000-$80,000, depending on experience.
- The successful candidate will work as part of a multidisciplinary team to deliver patient-centered care, ensuring the best possible outcomes for our patients.
- This role involves providing individualized goal-directed nursing care through the use of the nursing process, under the supervision of an RN or Clinical Manager.

Responsibilities:
- Providing quality care to assigned patients.
- Carrying out the plan of care for each patient, ensuring that the patient/caregivers are included in the care plan process.
- Ensuring documentation is completed, communicating via a note, the activities and needed actions of each visit to the RN.
- Implementing medical plans of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients.
Requirements:
- Licensed Practical Nurses (LPN) license.
- Prior clinical acute care nursing experience required. Prior Home Care Experience preferred.
- Valid Driver's License, Vehicle, and Insurance. Ability to successfully pass motor vehicle record check, child abuse, criminal record check, and adult abuse check required.
