About the Role:
Adoration Home Health and Hospice is seeking a skilled Physical Therapist to join our team in Lafayette, TN. As a Physical Therapist, you will make intermittent home-visits to our skilled nursing patients, providing personalized care and support to help them achieve their maximum level of independence and function.

Responsibilities:

  • Conduct thorough physical therapy evaluations and develop personalized treatment plans to meet the unique needs of each patient
  • Provide skilled physical therapy services, including exercises, stretches, and adaptive equipment recommendations
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Maintain accurate and timely documentation of services provided to ensure proper billing and reimbursement
  • Supervise Physical Therapist Assistants and Physical Therapy Aides as needed

Requirements:

  • Master's or Doctoral degree in Physical Therapy from an accredited program
  • Current, unrestricted license as a Physical Therapist in the state of operation
  • Current BLS or CPR certification
  • Minimum one year of experience in home health or skilled nursing setting
