Position Overview:
We are looking for a dedicated Client Care Coordinator who will oversee all aspects of care for both prospective and current clients.
Key Responsibilities:
- Answer incoming calls in a courteous and knowledgeable manner, directing them to the appropriate personnel.
- Effectively communicate with clients and their families regarding services and expectations.
- Conduct service inquiries and input prospect information into our management system.
- Prepare for Care Consultations, engaging with potential clients and their families to assess needs and propose tailored service plans.
- Ensure compliance of new client files with state regulations, both in physical and digital formats.
- Collaborate with healthcare providers and assistance programs to coordinate client care.
- Assist in scheduling for clients and caregivers.
Qualifications:
- High school diploma or equivalent required.
- Minimum of two years of relevant business experience or a combination of education and experience.
- One year of caregiving experience preferred to enhance client assessments.
- Valid driver's license is necessary.
Skills and Abilities:
- Understanding of policies and procedures established by the organization.
- Excellent oral and written communication skills, with the ability to listen effectively.
- Strong interpersonal skills and sound judgment.
- Ability to maintain confidentiality and meet deadlines.
- Proficiency in office software, including Excel and Word.
- Must be able to work independently and as part of a team, demonstrating professionalism and a positive demeanor.
Work Schedule:
- Monday through Friday, 8:00 am to 5:00 pm.
- On-call duties required, including evenings and weekends.
