VNS Health is seeking a compassionate and experienced Registered Nurse (RN) Manager to lead our Hospice team in providing end-of-life care to patients in need.
About the Role- Lead a dedicated interdisciplinary team in providing hospice care to patients and their families.
- Manage, coordinate, and facilitate the team's efforts to ensure high-quality patient care.
- Develop and implement clinical and quality improvement initiatives to improve patient outcomes and regulatory compliance.
- Ensure the use of an interdisciplinary approach to assess patients' medical, physical, social, emotional, and spiritual needs.
- Provide support and direction to patients' families in accordance with the patient's Interdisciplinary Plan of Care.
- Participate in Hospice Team meetings, Interdisciplinary group management meetings, family meetings, and patient calls.
- Ensure full regulatory compliance with Hospice Conditions of Participation and standards.
- Collaborate with Quality Improvement and Education to ensure professional practice standards are consistently met.
- License and current registration to practice as a Registered Professional Nurse in New York State.
- Bachelor's degree in nursing from an approved program accredited by the National League of Nursing or Health or Human Services field.
- Minimum three years of clinical nursing experience, preferably in a home care or hospice setting.
- Clinical management experience is preferred.
