As a Clinical Coordinator, you will be responsible for coordinating our systemwide Pharmacy and Therapeutics Committee, ensuring that our medication management practices are safe, effective, and aligned with best practices.

Responsibilities:
  • Coordinate Glycemic Management Programs across the health system
  • Work closely with Internal Medicine teams at Garnet Health Medical Center Middletown
  • Provide support and guidance on best practices and safe medication use
  • Coordinate the systemwide Pharmacy and Therapeutics Committee

Qualifications:
  • PharmD degree
  • Successful completion of a PGY1 Residency
  • Current and ongoing NYS Pharmacist License and Registration

Working Conditions:

This is a full-time position, with occasional on-call availability. You will work in a clean, well-lighted, heated or ventilated facility, with no routine exposure to hazards.
