Job Overview

Purpose of the Role:

The physician serves as the medical authority within the Interdisciplinary Group (IDG), overseeing the comprehensive medical management of hospice patients. This includes delivering care in home settings, inpatient facilities, emergency situations, and respite care associated with terminal illnesses. The physician is accountable for the medical treatment of hospice patients and acts as the primary medical consultant to the IDG, ensuring medical support is available during and after standard working hours, as well as conducting patient visits for medical evaluations and necessary interventions, whether in a facility or at home.

Key Responsibilities:

  • Certifies in writing that each patient considered for hospice admission or continued care is terminally ill, in compliance with relevant laws, regulations, and hospice policies.
  • Oversees the medical services provided to hospice patients, ensuring that care from attending physicians is complemented and that continuity of medical services is maintained, particularly in managing pain and symptoms associated with terminal conditions.
  • Records the progression of individual patients in end-stage diseases within their medical records as appropriate.
  • Maintains good standing medical staff privileges at facilities that provide short-term inpatient care for hospice patients.
  • Supervises all physician staff and any contracted physicians.
  • Ensures that qualified physicians participate in the initial creation and ongoing evaluation of the IDG care plan, which must be established prior to the initiation of hospice services.
  • Actively engages in IDG meetings, including updates to each patient's comprehensive assessment and care plan as frequently as required by the patient's condition and in accordance with applicable laws and regulations.
  • Participates in:
    • The formulation, execution, and assessment of clinical protocols;
    • Decisions regarding patient continuing care;
    • The development of procedures and oversight of short-term inpatient care, emergency services, and respite care;
    • The quality assessment and performance improvement program (QAPI);
    • Resolving clinical and ethical disputes or concerns involving patients and families;
    • Continuing education for the IDG, volunteers, and affiliated physicians and staff of associated inpatient facilities.
  • Acts as a physician for patients to address general medical needs, including pain and symptom management or other medical interventions when the attending physician is unavailable or has transferred responsibility for care.
  • Serves as a liaison to community physicians.
  • Completes additional tasks as assigned.
  • May access personal health information (PHI) necessary to perform the above duties, with access and disclosure defined by organizational policies.

Education & Experience Requirements:

  • Doctor of Medicine or Osteopathy degree.
  • Current, valid medical license in the state(s) where hospice care is provided.
  • Board certification in a relevant specialty is preferred.
  • Experience in hospice or palliative care is preferred.

Required Knowledge, Skills & Abilities:

  • Understanding of applicable laws, regulations, and contract provisions related to terminal illness certification for hospice admission or ongoing care.
  • Ability to collaborate effectively as a member of the Interdisciplinary Group.
  • Knowledge of and commitment to the philosophy of hospice care and palliative interventions.
  • Strong communication skills with clinical and non-clinical staff, patients, families, and the medical community.
  • Meets health requirements necessary for patient care.
  • Complies with federal, state, and organizational background check requirements.

Work Environment & Physical Requirements:

  • Work is typically conducted in an office environment.
  • Occasional local travel is required for patient visits.
  • May encounter communicable diseases, bloodborne pathogens, and other potentially hazardous materials, necessitating adherence to safety protocols and the use of protective equipment.
  • Must be able to stretch, bend, stoop, twist, stand, sit, walk, and reach as needed.
  • Availability for on-call hours may be required.
  • Must be able to lift and carry up to 50 pounds to perform daily job functions and related activities.
