WellBe Senior Medical is dedicated to providing exceptional geriatric care through a Physician Led Advanced Practice clinician model. Our focus is on serving the frail, poly-chronic elderly Medicare Advantage patients who often face significant barriers to accessing healthcare. We believe in delivering care directly to the patient, ensuring they receive comprehensive support throughout their healthcare journey, from home-based chronic and urgent care to hospital and palliative services.

Position Summary

The Engagement Nurse Practitioner plays a pivotal role in embodying the values and mission of WellBe in the delivery of care to our patients. This position requires adherence to state and federal regulations while providing compassionate, patient-centered care in the home environment. As an integral member of our interdisciplinary team, the Engagement Nurse Practitioner is responsible for onboarding eligible patients into the WellBe care model, conducting initial assessments, and ensuring seamless transitions to ongoing care.

Key Responsibilities

  • Conduct in-home visits to engage patients and assess their healthcare needs.
  • Collaborate with other healthcare professionals to manage the health and wellbeing of patients.
  • Provide educational resources to facilitate informed decision-making for patients and their families.
  • Perform comprehensive patient evaluations and maintain accurate documentation.
  • Address urgent care needs as they arise, ensuring timely interventions.
  • Work closely with the community team to develop individualized care plans.
  • Order and interpret diagnostic tests, coordinating follow-up care as necessary.
  • Prescribe medications and manage treatment plans in accordance with state regulations.
  • Ensure the accuracy of patient records and documentation at each visit.
  • Deliver high-quality care while adhering to company standards and metrics.
  • Serve as a resource for patients, providing ongoing support and guidance.
  • Utilize available resources to meet patient needs and refer to appropriate services.
  • Participate in interdisciplinary team meetings to enhance collaborative care.
  • Maintain active state licensure and national certification.
  • Uphold professional standards in patient care and documentation.
Qualifications
  • Master's Degree in Nurse Practitioner studies from an accredited program.
  • Active Registered Nurse and Nurse Practitioner licenses in the relevant state.
  • Current board certification (AANC or AANP).
  • BLS certification and active NPI number.
  • CPR certification.
  • DEA licensure or eligibility upon hire.
  • Experience in home care, primary care, hospice, or geriatrics is preferred.
  • Valid driver's license and access to a vehicle for patient visits.
Skills and Abilities
  • Ability to conduct home visits effectively.
  • Proficiency in Microsoft Office and Electronic Health Records.
  • Strong communication and interpersonal skills.
  • Experience in managing chronic and urgent medical conditions in older adults.
  • Commitment to teamwork, integrity, and ethical practice.
Work Environment

This position requires the ability to lift and transfer patients, as well as travel to various patient locations. Candidates should be prepared for physical demands associated with patient care.
