About the Role

HarmonyCares, a leading provider of home-based primary care, is seeking a skilled Registered Nurse to join our team. As a Nurse Care Specialist, you will play a vital role in delivering high-quality, coordinated healthcare to patients in the comfort of their own homes.

Key Responsibilities
  • Assess and Evaluate Patient Needs: Conduct thorough assessments to identify patient needs and develop personalized care plans that address their unique requirements.
  • Lead and Supervise Hospice Aide Staff: Direct and instruct hospice aides in providing exceptional patient care, ensuring that patients receive the highest level of service.
  • Collaborate with Clinical Leadership: Work closely with clinical leaders, attending physicians, and hospice physicians to ensure seamless care coordination and effective communication.
  • Participate in Quality Improvement Initiatives: Contribute to our Quality Assessment and Performance Improvement (QAPI) program by submitting data and participating in quality improvement initiatives.
  • Deliver Age-Friendly Care: Ensure that patient care is tailored to meet the unique needs of each patient, incorporating the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility).
Requirements
  • Current RN Licensure: Possess an active, unencumbered registered nurse license in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
  • Valid Driver's License: Maintain a valid driver's license and good driving record.
  • Excellent Nursing Judgment: Demonstrate the ability to make sound nursing judgments and assess patient needs effectively.
  • Experience with Microsoft Office: Possess experience with Microsoft Office and ability to work efficiently in a field setting.
