Key Responsibilities:
- Coordinate pre and post-transplant patient activities, including scheduling appointments, facilitating plans of care, and coordinating with transplant physicians and other healthcare professionals.
- Develop and maintain relationships with patients, families, and donors, providing education and support throughout the transplant process.
- Collaborate with the transplant team to ensure seamless coordination of patient care, including coordinating with the hospital unit personnel in the discharge of transplant patients and donors.
- Assist in obtaining medications and supplies from the appropriate source according to the patient's insurance coverage.
- Prepare and present cases to the Selection Committee, and place patients on the national waiting list for transplantation.
- Coordinate ongoing care of inpatients and outpatients waiting for transplant, and triage patients during the entire transplant process for any medical and psychosocial issues.
- Be available by beeper and by phone in order to provide immediate assessment and delivery of care in a timely and cost-effective manner.
- Collaborate with Office Coordinators and Financial Counselors in obtaining insurance clearance for patients prior to transplant procedure.
- Coordinate informational packets, which are sent to patients prior to transplant, and review information with patients upon their arrival and answer any questions regarding the transplant information and transplant schedule.
- Coordinate and provide educational sessions for patients, families, donors prior to transplant, and collaborate with the Organ Procurement Organization to facilitate the organ procurement surgery and subsequent transplant surgery.
Requirements:
- Minimum of two years RN experience and one year acute care hospital experience including disease-related experience.
- Basic Life Support (BLS) Certification.
- Ability to multitask and prioritize, thrive in a team environment, and demonstrate a strong work ethic.
- Preferred qualifications include at least 2 years of RN experience working in the ICU or ED, Bachelor's Degree, and ability to provide care with compassion and empathy.
Physical Requirements:
- Requires full range of body motion, manual finger dexterity, hand and eye coordination.
- Requires working under stressful conditions and working irregular hours.
- Must be willing to be on call 24 hours per day on a rotating basis.
Personal Protective Equipment:
- Follows Standard Precautions using personal protective equipment as required for procedures.
