Job Summary

At Embrace Hospice & Home Health, we are seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our team. As a Hospice Care Professional, you will play a vital role in providing personal care services to patients in their homes, supporting their physical, emotional, and spiritual needs.

Responsibilities

• Provide personal care services, including bathing, dressing, and grooming, as assigned by the patient's care plan.
• Assist patients with daily living activities, such as feeding, toileting, and ambulation, as needed.
• Monitor patients' vital signs, including temperature, pulse, and blood pressure, and report any changes to the Case Manager.
• Maintain a clean and safe environment for patients, including their living areas and equipment.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Participate in quality assessment and performance improvement activities to enhance patient care and services.

Qualifications

• Meet the training requirements of the State, including a minimum of 75 hours of classroom and supervised practical training.
• Possess a minimum of 6 months of experience in providing personal care services, including experience working with terminally ill patients.
• Demonstrate strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
• Possess a calm and compassionate demeanor, with the ability to provide emotional support to patients and families.
• Maintain current CPR certification and adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
