Position Overview:
Join our compassionate team as a Practical Nursing Specialist. This role offers a daytime schedule with no night shifts, on-call duties, or weekend work, allowing you to maintain a healthy work-life balance.

Key Responsibilities:
As a Practical Nursing Specialist, you will play a crucial role in delivering high-quality nursing care and support to individuals with complex medical and rehabilitative needs. Your responsibilities will include:

  • Monitoring the health status of individuals and ensuring the delivery of quality services.
  • Administering medications and maintaining accurate documentation while supervising staff in medication administration.
  • Implementing prescribed medical treatments and personal care services as per physician's directives.
  • Conducting health assessments and integrating healthcare services into each Individual Service Plan (ISP), responding proactively to any changes in condition.
  • Providing training to staff on essential topics such as infection control, medication administration, and universal precautions.
  • Acting as a liaison and advocate among medical professionals and external agencies to coordinate comprehensive care.
  • Educating individuals on self-care topics, including medication management, health education, and wellness initiatives.

Qualifications:
To be successful in this role, you should possess:

  • An Associate's Degree in Nursing or a related field, or a valid state certification.
  • A minimum of one year of nursing experience with the target population.
  • Current LPN or LVN licensure in your state of residence.
  • A valid driver's license, car registration, and auto insurance.
  • Additional certifications as mandated by state regulations, including CPR/FA and HIV/AIDS training.
  • Strong communication skills to effectively engage with staff, individuals served, and their families.
