Key Responsibilities- Conduct routine checks on residents to ensure their personal care needs are met in accordance with their wishes.
- Review patient care plans daily to identify any changes or updates.
- Communicate effectively with the nurse supervisor and other healthcare team members to ensure seamless care delivery.
- Assist residents with daily activities, including preparation for social programs and transportation to appointments.
- Maintain a warm and welcoming environment that promotes resident comfort and well-being.
- Ensure resident rooms are ready for receiving and help residents feel at ease.
- Participate in facility surveys and maintain confidentiality of resident care information.
- Effectively communicate resident information to charge nurses, director of nursing, and/or administrator.
- High school diploma or equivalent required.
- Current licensure as a Certified Nursing Assistant in this state.
