Job Summary

Amergis is seeking a highly skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will be responsible for providing and documenting skilled one-on-one nursing care in accordance with the physician-ordered individualized patient care plan.

Key Responsibilities:
  • Conduct initial assessments of patients and families to determine home care needs.
  • Provide complete physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs and initiate revisions as necessary.
  • Develop and implement individualized patient care plans in collaboration with the healthcare team.
  • Administer medications and treatments as prescribed by the physician.
  • Provide healthcare instructions and education to patients and families as needed.
  • Identify discharge planning needs and ensure a smooth transition of care.
  • Adhere to home health agency policies and protocols.
Requirements:
  • Graduate of an accredited school of nursing.
  • Currently licensed as a Registered Nurse in the state where the RN will practice.
  • Compliance with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program.
  • Current CPR certification.
  • TB questionnaire, PPD, or chest x-ray as required.
  • Current Health certificate.
  • Meet all federal, state, and local requirements.
  • Successful completion of new hire training.
  • Understanding of patient confidentiality and HIPAA requirements.
  • Strong communication and computer skills.
