Job SummaryThis is a full-time opportunity for a Registered Nurse to provide medical care to patients in the comfort of their own homes. The ideal candidate will have experience in Home Health and be able to work independently with minimal supervision.
Responsibilities- Conduct thorough nursing assessments of patients and document findings in their medical records.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide medication administration and education to patients and their families.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive care.
- Monitor patient progress and adjust care plans as needed.
- Current RN License and BLS Certification required.
- At least 1 year of experience as a Registered Nurse and 6 months of recent Home Health experience preferred.
- Valid driver's license and auto insurance required for frequent travel to patient homes.
