About the Role:We are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will conduct in-home nursing visits for our clients, providing high-quality care and support to those in need.
Responsibilities:- Conduct in-home nursing visits for clients, providing physical assessments, ensuring home safety, and overseeing skilled and non-skilled caregivers.
- Provide case management to assigned clients, directing a team of RNs, LPNs, and caregivers in the delivery of care.
- Plan, implement, and evaluate care provided, participating in client care conferences as needed.
- Serve as a local on-site clinical resource, providing support to ensure client's home care needs are met.
- Assist and collaborate with staffing coordinators regarding staffing and scheduling of personnel within scope of practice, competencies, client needs, and complexity of home care.
- Graduate of a state-approved school of professional registered nursing.
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
- Provide direct care and case management of assigned clients.
- Competitive hourly rate of $00 plus shift differentials, where applicable.
- Medical, dental, and short-term disability insurance.
- Flexible spending account (FSA) and health savings account (HSA).
