Compensation: The annual salary for this position ranges from $88,320 to $114,720, depending on experience and location.
About the Role: As a Home Healthcare Case Manager, you will be responsible for conducting in-home nursing visits, assessing clients' needs, developing care plans, and collaborating with caregivers and other healthcare professionals.
Key Responsibilities:
- Conduct thorough assessments of clients' physical and emotional needs
- Develop and implement comprehensive care plans that meet clients' unique requirements
- Collaborate with caregivers and other healthcare professionals to ensure seamless care delivery
- Provide education and support to clients and their families on health management and disease prevention
- Maintain accurate and up-to-date records of client interactions and progress
Requirements:
- Graduate of an accredited school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two years nursing experience, including one year in home care or a closely related field
- One year of supervisory and/or case management experience preferred
