As a Community Health Worker, you will be responsible for providing clinic-based health education and outreach services to community clients, addressing chronic disease and prevention techniques.
Key Responsibilities- Promote health benefit programs and clinical services to community clients
- Organize community meetings and focus groups to engage with community members
- Develop and complete a community survey and needs assessment to determine additional services and eligibility for health programs
- Outreach to community residents to promote benefits for the uninsured
- Provide clinic-based health education services for clients and conduct community-based education and outreach addressing chronic diseases and prevention techniques
- Travel to different sites as assigned to provide services
- Attend and participate in staff meetings and training as required by supervising staff
- Maintain records of community outreach, education, and meetings and complete data entry as needed for progress reports
- Prepare reports, collect data, evaluate projects, and submit appropriate information to supervisor in a timely manner
- Bachelor's degree and two (2) years of experience providing information, education, intervention, and/or referral services to culturally diverse populations, or
- Five (5) years of experience working in a related field
- Public speaking ability
- Valid California driver's license, proof of auto insurance, and personal transportation
The work is performed in an office environment within a clinic setting. Involves frequent contact with staff and the public. Work may be stressful at times. Contact may involve dealing with upset people.
