Job Description

This role plays a vital part in our team, conducting thorough clinical assessments, developing effective health strategies for patients, and maintaining accurate records. Key responsibilities include:

Clinical Assessment and Management
  • Performing independent history-taking and physical examinations.
  • Identifying patient and family needs regarding preventative care, disease entities, medications, and therapeutic regimens.
Treatment Planning and Implementation
  • Ordering and interpreting therapeutic regimens.
  • Collecting and interpreting patient data.
  • Consulting with physicians as needed.
Patient Education and Support
  • Developing problem lists from assessment data.
  • Establishing treatment goals considering patient physical and emotional status.
  • Planning remedial actions within established protocols or in collaboration with physicians.
  • Collaborating with physicians and healthcare teams to address patient needs.
Communication and Collaboration
  • Monitoring patient caseloads in collaboration with physicians.
  • Seeking assistance from healthcare teams when necessary.
  • Participating in patient care conferences and implementing care plans.
  • Coordinating patient care to review reports, consultations, and recommendations from other healthcare professionals.
Evaluation and Improvement
  • Evaluating plan effectiveness based on collected data.
  • Revising and updating care plans to meet changing patient needs.
