RN, Case Manager, University Hospital, Part Time Weekends

We are seeking a skilled RN Case Manager to join our team at UofL Health - Care Management. This is a part-time position working weekends, 7a-7p.
Responsibilities- Conduct timely initial admission assessments to promote optimal management of clinical resources.
- Validate patient demographics and payor source.
- Implement, reassess, and evaluate patient condition and coordinate plan of care daily.
- Develop patient-specific plan of care based on individual needs and conversations with providers.
- Coordinate discharge needs with internal and external providers while protecting patient information.
- Perform timely and professional documentation.
- Advocate for patient/family needs in a respectful and confidential manner.
- Serve as a liaison or facilitator for the patient and interdisciplinary healthcare team.
- Educate the healthcare team about the role of the RN Case Manager.
- Refer cases to manager/director of care coordination, CAO, or medical director when necessary.
- Ensure delivery of Discharge Medicare Important Message (IM) and Medicare Outpatient Observation Notice (MOON), where applicable.
- Acknowledge, address, and complete Consults to Case Management on respective units in EMR.
- Work collaboratively with social workers to identify social and financial barriers and community resources.
- Complete referrals to Social Worker for patients with high-risk indicators.
- Attend monthly departmental staff communications meetings and serve as an active member of committees as needed.
- Enhance professional growth and development through participation in educational programs and workshops.
- Maintain competency in care management principles and educate physicians, patients, and staff on payors, financial issues, documentation, and potential compliance issues.
- Serve as a resource to physicians for clinical management and financial issues.
- Maintain compliance with all company policies, procedures, and standards of conduct.
- Comply with HIPAA privacy and security requirements to maintain confidentiality at all times.
- Licensed or eligible for license in Kentucky.
- Basic Life Support (BLS) accredited by the American Heart Association (AHA).
- Bachelors degree from accredited college or University or RN with a Bachelors Degree in Business or Health Care Administration preferred.
- Must have 2-3 years experience as an RN plus utilization management or discharge planning/case management experience.
- Minimum two years experience performing utilization review and/or quality assurance activities in an acute care facility or HMO/PPO/PRO preferred.
