Requirements:

- You must be a Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.">
- You must be Board Certified in Internal Medicine or Family Medicine.">
- You must have an active, non-probationary, unrestricted State License and an Active DEA license.">
- You must be authorized to work in the United States.">
