Job Description
Senior Community Care of KY-PACE seeks a skilled Physical Therapist to provide physical therapy services to program participants. As a member of our interdisciplinary team, you will conduct comprehensive assessments, develop treatment plans, and provide treatment to achieve identified goals.

Responsibilities
* Develop and implement individualized treatment plans for participants
* Coordinate with the interdisciplinary team to ensure seamless care delivery
* Maintain accurate and detailed records of participant progress and treatment
* Communicate effectively with participants, family members, and healthcare providers

Qualifications
* Master's degree in physical therapy or related field
* Current licensure as a Physical Therapist in the state of practice
* Minimum one year's experience working with older adults or individuals with complex medical conditions

Working Environment
The work environment for this role may include exposure to bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases, and other conditions common in a healthcare setting. You must be able to handle physically demanding tasks and maintain a safe working environment.
