Role Overview
We are seeking a dedicated and compassionate LPN/RN to join our team at Trilogy Health Services. As a key member of our care team, you will be responsible for providing high-quality patient care and promoting a positive and supportive environment for our residents.
Responsibilities
- Develop and implement individualized care plans for each resident, taking into account their unique needs and preferences.
- Administer medications and treatments as prescribed by physicians, and maintain accurate records of patient care.
- Provide emotional support and comfort to residents and their families, and promote a sense of community and belonging within our facility.
- Collaborate with other healthcare professionals to ensure seamless and effective care delivery.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
- Current LPN or RN licensure in the state of employment.
- Experience in long-term care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Flexibility and adaptability, with a willingness to work varied shifts and respond to changing circumstances.
