Job Description:

As a Nurse Practitioner, you will be responsible for ensuring that all activities align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Hospice team.

You will prevent unwanted hospitalizations by meeting patient and family needs in the home.

You will prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.

You will comply with applicable laws and regulations with respect to Collaborative Agreements.

You will complete accurate billing and coding activities for all patient encounters according to CMS guidelines and initiate reimbursement for medically necessary visits in accordance with applicable law and regulation.

You will provide training and continuing education for staff.

You will assist in the development of clinical practice guidelines/standards in support of quality palliative care.

You will adhere to the practice of confidentiality regarding patients, families, staff, and the organization.

You will collaborate/communicate with Medical Director, attending physician, branch leadership, and IDG.

You will perform other duties/projects as assigned.

Requirements:
  • Masters Degree in Nursing and certification through a national Nurse Practitioner Licensing board.
  • 3 years clinical nursing experience
  • 1 year post-graduate clinical nursing experience in the hospice and palliative care field.
  • Prior experience in Family Medicine, Geriatrics, and psychiatry preferred.
  • Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
  • Board Certification in Palliative Care Preferred.
  • Active DEA license.
  • Able and willing to travel up to 50% as needed to support business and patient needs.
