Maternal Health Case Manager Job Description

As a Maternal Health Case Manager, you'll play a vital role in promoting healthy pregnancy, delivery, and postpartum outcomes for mothers and babies. Your expertise will help address barriers to care and social determinants of health, ensuring our members receive the support they need to thrive.

Key Responsibilities:
  • Provide evidence-based education to promote healthy pregnancy, delivery, and postpartum outcomes
  • Develop and implement care plans to address member needs and goals
  • Collaborate with obstetric and other providers to ensure seamless care coordination
  • Support members in accessing value-added benefits, including Medicaid postpartum coverage
  • Conduct mental health and substance use disorder screenings, with referrals to treatment when necessary

Requirements:
  • Current, unrestricted independent licensure as a Registered Nurse in Kansas
  • 2+ years of experience working within the maternal and child health population
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Ability to travel up to 75% of time in assigned region to visit members in their homes and/or other settings
