Position Overview

The RN Case Manager is responsible for delivering skilled nursing care to patients requiring professional nursing services. This role involves educating and supervising family members and other nursing staff, ensuring that care is provided in accordance with physician directives.

Key Responsibilities

  • Deliver and coordinate high-quality patient care while managing a full caseload and achieving productivity targets.
  • Efficiently manage patient care to achieve positive outcomes, regardless of the payer source.
  • Oversee the overall care plan and outcomes, including supervision of other nursing staff (LPN, HHA).
  • Conduct patient admissions and manage care by:
    • Gathering medical histories relevant to the current condition from patients and family members.
    • Performing physical examinations, including vital signs and assessments of daily living activities.
    • Assessing the patient's environment and available support from family members.
    • Determining the need for home health aide services and their frequency.
    • Interpreting agency services to patients and families as part of care planning.
    • Ensuring compliance with regulatory requirements and understanding various insurance policies.
    • Implementing infection control standards in nursing practice.
    • Developing and executing nursing care plans.
    • Providing skilled nursing services as outlined in care plans, including treatments and preventive measures.
    • Monitoring patient conditions and reporting any changes to the physician.
    • Educating and guiding patients and caregivers regarding nursing care needs.
    • Supervising and evaluating the care provided by Home Health Aides.
    • Maintaining accurate documentation of services rendered and patient progress.
  • Other duties as assigned.

Educational Requirements

  • Associate's degree in Nursing is required.
  • Bachelor's degree in Nursing is preferred.

Qualifications

  • Current, valid RN license issued by the NYS Department of Education is required.
  • Willingness to learn about certified home care regulations and payer guidelines.
  • Essential skills include:
