Salary: The estimated annual salary for this position is around $55,000-$65,000, depending on experience and qualifications.

About the Role:

This field-based position requires a strong community health background and excellent communication skills. You'll be working closely with our clinical care management team to manage complex members in the community, locate and engage them, and provide education and coaching to support their self-management of needs and health promotion.

Your Responsibilities:

  • Partner with our clinical care management team to develop individualized plans of care that focus on the member's definition of success
  • Identify and overcome barriers to goal achievement, including socioeconomic barriers
  • Locate and engage members in the community through face-to-face or telephonic screenings
  • Research, locate, and make referrals to community resources to address member-specific care or socioeconomic needs
  • Provide education and coaching to support HEDIS/STAR gap closure and empower members to manage their care

Requirements:

  • High School Diploma/GED (or higher)
  • 3+ years of experience working within the Community Health setting in a field-based healthcare role
  • 2+ years of experience working within the local community, with demonstrated knowledge of culture and values within the community and familiarity with available resources
  • Intermediate knowledge of Medicare and Medicaid benefits
  • Intermediate proficiency with Microsoft applications
  • Access to reliable transportation and ability to travel up to 25% of the time

Preferred Qualifications:

  • Bachelor's Degree (or higher) in Social Work or related subject
  • LPN (Licensed Practical Nurse) Licensure or CNA/HHA
  • Bilingual in Spanish
  • Community Health Worker (CHW) Accreditation
  • Experience working in team-based care and managed care environments
