Position Overview
The Licensed Practical Nurse (LPN) plays a crucial role in delivering and documenting proficient nursing care under the guidance of a Registered Nurse. This role involves adhering to established care plans and physician directives for each patient while maintaining confidentiality and professional boundaries at all times.

Key Responsibilities
  • Deliver and coordinate high-quality patient care in accordance with physician orders.
  • Continuously monitor and evaluate patient conditions, reporting any changes to the supervising nurse or physician as necessary.
  • Document all nursing activities, assessments, and care coordination promptly and accurately.
  • Participate in the development, implementation, and revision of nursing care plans.
  • Take appropriate nursing actions based on patient assessments to achieve desired health outcomes.
  • Recognize and respond to changes in patient needs, implementing interventions to mitigate risks.
  • Maintain compliance with Aveanna policies, state and federal regulations, and the Nurse Practice Act.
  • Utilize infection control measures to safeguard both staff and patients in accordance with OSHA standards.
  • Educate patients and their families about disease processes, self-care techniques, and preventive strategies.
  • Engage in ongoing professional development to maintain nursing competencies.

Qualifications
  • Graduation from an accredited nursing program.
  • Current, unrestricted state license as a Licensed Practical Nurse.
  • Valid CPR certification.
  • Proven proficiency in clinical assessments and adherence to nursing policies and procedures.

Preferred Experience
  • At least six months of recent nursing experience in a clinical setting.
  • Experience in home health care.

Physical Requirements
  • Ability to communicate effectively in English, both verbally and in writing.
  • Capability to lift up to 50 pounds and assist with patient mobility.
  • Ability to perform physical tasks such as prolonged walking, standing, bending, and climbing stairs.

Work Environment
Must be adaptable to various environments, which may include exposure to allergens and infectious materials. This role may involve potential exposure to blood and bodily fluids.
