Gentiva Hospice is seeking a skilled RN Clinical Liaison to join our team. As a key member of our organization, you will play a vital role in educating healthcare professionals about hospice services and providing end-of-life solutions that prioritize clinical excellence, compassion, and dignity.

Key Responsibilities
  • Intake and Referral Process: Ensure the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
  • Admissions Process: Complete the admissions process on referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions.
  • Patient Advocacy: Serve as a patient advocate in evaluating and developing plans of care and obtaining informed consent for hospice.
  • Community Outreach: Call on multiple account types, including SNFs, ALFs, hospitals, and physicians, to offer in-services and education on the benefits of hospice.
Requirements
  • Hospice Heart: Demonstrate a compassionate, kind, and empathetic approach to clients, patients, and their families.
  • RN License: Possess a valid RN license in the state(s) of employment.
  • Healthcare Experience: Minimum of 2 years of experience in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
