Psychiatric Nurse Specialist

Our client, a leading provider of behavioral health services, is seeking a highly skilled Registered Nurse to join their team in Philadelphia, PA.
Job Summary:The successful candidate will be responsible for coordinating the behavioral and physical health care needs of service participants, monitoring medication, and providing psychiatric nursing services consistent with physician orders and standard nursing practices.
Key Responsibilities:- Assist team members in coordinating behavioral and physical health care needs of service participants
- Complete health and biopsychosocial assessments for every active participant
- Maintain a record of medical history and on-going medical and treatment records
- Service documentation must be submitted in a timely fashion
- Consult with team members regarding development and implementation of the health care needs and goals
- Coordinate with team members routine medical, vision, lab and dental appointments
- Ensure a mechanism for reporting and follow-up results
- Attend team meetings regularly to review medications, develop emergency intervention plans, review health care needs, report on participants' responses to treatment, and coordinate appointments
- Consult with community agencies and families to maintain coordination in the treatment process
- Under the direction of the Lead Psychiatric Nurse and in collaboration with other departmental nurses, manage pharmaceuticals and medical supplies
- Monitor participant's medication
- Coordinate the administration of injections, as needed
- Participate in discharge planning for participants from acute or extended care facilities to provide continuity of medical and psychiatric care
- Complete required documentation
- Develop, review, and revise forms, procedures and standards of care according to accepted professional practice
- Conduct Quality Assurance
- Review of Health Care Section of medical record
- Maintain knowledge of current trends and developments through review of literature and periodicals, attendance at seminars and through contacts with other professionals
- Must meet funding source and license requirements for continuing education
- Provide education services to participants, community agencies and non-medical staff including medication, diagnosis and interventions
- Perform other duties and responsibilities as they relate to the overall enhancement of program service delivery
- Board Certified Licensed RN in the State of PA
- Prior experience in servicing community mental health facilities and their psychiatric needs
- Comprehensive knowledge of psychiatric, clinical and psychosocial rehabilitation practices
- Understanding of the Assertive Community Treatment (ACT) model of service
- Must be able to communicate both verbally and in written form with every level of the behavioral health system and maintain effective professional relationships with associates in the Agency/community at large
- Very strong organization and analytical skills
- Must be available for local travel; valid driver's license with an acceptable driving record required
