Job Overview

Position: Manager, Community Health Nursing

Location: Ridgewood Queens, New York

Schedule: 8:30 AM - 5:00 PM

Weekly Hours: 37.5 Hours

Role Summary: The Nursing Care Operations Manager is responsible for upholding nursing excellence through advanced professional education aimed at delivering exceptional care. This role involves collaboration with the management team and various departments to oversee daily operations, ensuring the organization meets its objectives and mission.

Key Responsibilities:

  • Guide and implement best practices to enhance experience, satisfaction, and retention.
  • Develop personalized care plans to optimize outcomes for all participants.
  • Facilitate Interdisciplinary Team (IDT) meetings and manage Service Delivery Requests (SDR).
  • Act as the on-call manager during non-operational hours, handling follow-ups and documentation.
  • Generate necessary Clinical Delivery reports.
  • Represent the Community Health Nurse (CHN) in IDT meetings as required.
  • Balance staffing and resources, ensuring adequate coverage and assignments.
  • Coordinate care with IDT and other necessary parties based on participants' healthcare needs.
  • Collaborate with clinicians to review assessments and ensure accuracy.
  • Investigate incidents and ensure timely documentation according to regulations.
  • Maintain complete and accurate participant documentation.
  • Review participant charts for quality assurance and address deficiencies.
  • Provide care management services across various sites, collaborating with the Transitions in Care Team.
  • Coordinate long-term care services as necessary.
  • Monitor care management activities and implement changes as needed.
  • Stay informed on health-related issues and clinical practices.
  • Contribute to service and policy improvements.
  • Assist Human Resources with interviews, coaching, and staff development.
  • Evaluate performance and competency of direct reports, providing necessary feedback.
  • Ensure compliance with organizational policies and regulations.
  • Address employee grievances as they arise.

Qualifications:

  • Degree from an NLN accredited nursing school (RN, BSN required or at least 5 years of experience in a healthcare setting).
  • Successful completion of an accredited Nursing Program is required.
  • A Baccalaureate degree is highly desired.
  • Completed NYSED-approved infection control coursework.
  • Minimum of 8.0 education contact hours, including infection control coursework.
  • Case Management Certification recommended within 2 years of employment.
  • Minimum of 2 years of administrative experience in management within a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, critical care, or nursing home experience.
  • Customer service experience is required.
  • Managed long-term care insurance experience beneficial.
  • Minimum of 1-year experience working with the elderly.
  • Supervisory experience is required.
  • Current active and unrestricted license and registration in New York State required as a Registered Nurse.
