Job Summary

We are seeking a highly skilled CLINICAL CASE MANAGER to join our team at Innovative Systems Group. As a Clinical Case Manager, you will be responsible for managing clinical cases for customers on government health care programs.

The ideal candidate will have 3 years of direct clinical care experience as a Registered Nurse (RN) or Licensed Professional Counselor (LPC), with a strong background in analyzing and providing data for regulatory reports. You will also possess excellent verbal and written communication skills, including the ability to discuss medical needs with members and interface with internal staff/management and external vendors and community resources.

Key Responsibilities:

  • Manage clinical cases for customers on government health care programs
  • Analyze and provide data for regulatory reports
  • Perform assessments, care planning, and care coordination in the member/family home and telephonically
  • Represent the company to members

Requirements:

  • Registered Nurse (RN) or Licensed Professional Counselor (LPC) with 3 years of direct clinical care experience
  • Current, valid, unrestricted license in the state of operations
  • Plus 3 years wellness or managed care experience presenting clinical issues with members/physicians
  • Knowledge of the health and wellness marketplace and employer trends
