The Shelter Case Manager role plays a vital part in our mission to create positive change.
Job OverviewThis position involves meeting with clients to establish comprehensive rehousing plans. The goal is to promote self-sufficiency and long-term housing stability.
Key Responsibilities- Conduct regular meetings with clients to discuss progress and set goals.
- Provide written notices according to EOHLC Uniform Shelter Rules and program requirements.
- Address barriers to housing, such as credit scores and past non-payment issues.
- Collect necessary documents and maintain accurate participant files.
- Create budgets for housing sustainability.
- Document case management efforts and services through detailed case notes.
- Promote affordable housing options and collaborate with other department staff.
- An associate's degree in Human Services or a related field.
- 1 year of experience in case management, focusing on pre-tenancy and tenancy-sustaining services.
- Strong interpersonal skills and knowledge of community resources.
- Ability to work effectively in a team environment.
- Demonstrated success in case management functions or eligibility determinations.
- A competitive hourly wage of $24-$27 per hour.
