About the Role:We are looking for a skilled Physical Therapist to work with our pediatric patients, providing a full range of care to help them achieve their goals and improve their quality of life.
Responsibilities:- Deliver physical therapy services to patients in their homes, following prescribed physician orders and adhering to confidentiality standards and professional boundaries.
- Conduct thorough evaluations to define physical therapy needs, goals, progress, and expected outcomes, in conjunction with referring physician's Plan of Treatment.
- Participate in coordination of services with healthcare team members, physicians, parent/guardians, nurses, and other staff.
- Maintain effective communication between staff, healthcare team members, and family.
- Supervise other personnel (PTA, HHA) as applicable.
- Complete OASIS assessments where allowed by state professional practice and regulation.
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
- Current, unrestricted state license as a Physical Therapist in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
- One (1) year of recent experience as a Physical Therapist.
- Pediatric experience preferred.
