AmeriHealth Caritas is seeking a skilled and compassionate Healthcare Navigator to join our Long Term Services and Support (LTSS) team in the Southwest Region of Pennsylvania. This role involves working with Medicaid-eligible individuals, identifying their long-term care needs, and coordinating access to necessary services.

About the Role:

  • Conduct face-to-face assessments to evaluate participants' needs, including physical health, behavioral health, social services, and long-term support services.
  • Develop and implement personalized care plans to ensure participants receive the necessary services and supports.
  • Collaborate with healthcare professionals, community agencies, and family members to ensure seamless coordination of care.
  • Maintain accurate records and reports to track participant progress and service delivery.

Requirements:

  • Bachelor's degree in social work, psychology, or related field preferred; 3+ years of social service or healthcare experience acceptable in lieu of degree.
  • Experience working with vulnerable populations, particularly those requiring personal care services.
  • Familiarity with the home and community-based service system and ability to navigate it effectively.
  • Excellent communication and interpersonal skills to build strong relationships with participants, families, and healthcare providers.
