Care Navigator

We are seeking a dedicated and compassionate Service Coordinator to join our team and make a difference in the lives of our members.
In this role, you will have the opportunity to work with a diverse group of individuals, providing them with the support and resources they need to achieve their goals and maintain their independence.
Key Responsibilities- Conduct thorough assessments to identify participant's needs and develop personalized care plans.
- C-coordinate and arrange for necessary LTSS services and other Covered Services.
- Build relationships with healthcare professionals, community agencies, and government organizations to ensure smooth delivery of services.
- Provide education and support to participants and their families on available resources and service options.
- 3+ years of experience in social services or related healthcare field.
- Valid driver's license and reliable automobile transportation.
- Ability to travel up to 90 minutes from home.
- Excellent communication and interpersonal skills.
- Bachelor's degree in social work, psychology, or related field preferred.
The estimated annual salary for this role is $60,000 - $70,000, depending on experience.
