Job Overview

Brio Living Services is seeking a skilled RN Clinical Coordinator to join our team. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care and collaborating with our interdisciplinary team.

Key Responsibilities:

  • Participate in daily team meetings to discuss patient care and develop care plans
  • Conduct periodic health assessments to identify patient needs and develop strategies for care
  • Assess acute needs and illness in the clinic, participant's home, and facilities, and develop plans to address these needs
  • Develop and implement advanced care plans to meet patient needs
  • Provide patient and caregiver education to promote healthy outcomes

Requirements:

  • RN-Nursing Diploma or Degree from an accredited School of Nursing or University
  • Current license to practice as an Registered Nurse (RN) in the State of Michigan
  • Minimum of one year of experience working with the frail elderly, or long-term care population
  • Previous home care experience or long-term care experience is preferred
