We are seeking a dedicated and compassionate RN Care Manager to join our Sunflower Health Plan team. As a key member of our care management team, you will play a vital role in delivering high-quality, cost-effective healthcare outcomes for our members.
Position Purpose:The RN Care Manager will be responsible for developing, assessing, and facilitating complex care management activities for our members. This includes:
- Conducting comprehensive assessments to identify member needs and barriers to care
- Developing personalized care plans and education for members and their families
- Collaborating with providers to identify resources and specialists to address member needs
- Providing ongoing follow-up and monitoring of member status and progress towards care plan goals
- Facilitating care management and collaborating with providers to ensure timely access to needed care or services
To be successful in this role, you will need:
- A degree from an accredited school of nursing or a Bachelor's degree in Nursing and 2-4 years of related experience
- State licensure and/or compact state licensure as a Registered Nurse
