Healthcare Navigator with NC Medicaid Expertise

The Tammy Lynn Center (TLC) seeks a highly motivated and detail-oriented Healthcare Navigator to support members in accessing essential services and resources. This dynamic role requires strong communication skills, a keen understanding of NC Medicaid services, and the ability to work collaboratively with care teams.
In this position, you will:
- Collaborate with Care Managers to conduct outreach and engagement activities, ensuring members receive comprehensive support and address unmet health-related needs.
- Link members to vital social services and community resources for housing, food, employment, and other critical areas, promoting self-sufficiency and improved health outcomes.
- Track referrals for community-based resources and social support services, monitoring progress and closing gaps in care.
- Assist members in scheduling appointments, arranging transportation, and facilitating prescription fulfillment, among other tasks.
You will also participate in care team case conferences, team huddles, meetings, and planning activities, contributing your expertise to ensure seamless service delivery. As a key member of our team, you will help identify members' needs through ongoing contact and support activity, develop educational materials, and support member engagement in health promotion activities.
To succeed in this role, you must possess:
- A high school diploma or equivalent, with at least two years of experience working directly with vulnerable populations or in a related field.
- Demonstrated knowledge of NC Medicaid services, either through lived experience or professional training.
- Excellent communication and interpersonal skills, with the ability to build rapport with members and care teams.
