Key Responsibilities
  • Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that address the physical, emotional, and social needs of our patients.
  • Provide advanced illness management and palliative care services, including diagnosis, treatment, and follow-up care, under the direction of our Professional Services Corporation Physician.
  • Work closely with patients, families, and caregivers to ensure that their needs are met and that they receive the highest level of care possible.
  • Participate in evening and weekend call rotations, providing on-call services in a clinically competent and responsive manner.
  • Prescribe medications, including controlled substances, as delegated and licensed, and order laboratory, imaging, and other diagnostic tests as needed.
  • Communicate clinical findings and treatment plans to our PSC Physician and other members of the care team, as needed.
  • Maintain accurate and detailed records of patient care, including clinical findings, treatment plans, and outcomes.
Requirements
  • Master's degree in Nursing from an accredited school of nursing.
  • Minimum of two years of experience as a Nurse Practitioner, with a focus on Advanced Illness Management and Palliative Care.
  • Current license as a registered nurse and Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician, as required by state law.
  • Part B Provider certification and Federal DEA license.
  • ACHPN certification in Hospice and Palliative Care, preferred.
