We are seeking a skilled and compassionate Geriatric Care Specialist to join our team at Geriatric Consultants, PLLC. As a Primary Clinician in our post-acute setting, you will be responsible for providing high-quality patient care while maintaining autonomy and independence to support their needs.

As a member of our team, you will work collaboratively with skilled nursing center leadership to meet specific clinical goals and priorities. You will also work closely with Medical Directors and/or Attending Physicians to provide patient-specific medical care.

Responsibilities:

  • Provide primary clinical care to patients in our post-acute setting
  • Work collaboratively with skilled nursing center leadership to meet clinical goals and priorities
  • Collaborate with Medical Directors and/or Attending Physicians to provide patient-specific medical care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct regular assessments and evaluations to ensure optimal patient outcomes

Requirements:

  • Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • Experience working in a post-acute setting
  • Strong clinical skills and attention to detail
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
