Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Clatsop Care Health & Rehabilitation Center. As a LPN, you will play a vital role in providing high-quality patient care services to our residents.
Key Responsibilities- Participate in the planning and delivery of patient care services under the supervision of an RN.
- Direct patient care activities in the absence of an RN.
- Function within the scope of practice for a Licensed Practical Nurse in the State of Oregon.
- Maintain the confidential nature of the work environment.
- Participate in the planning and organization of patient care.
- Coordinate with other departments and care providers to deliver patient care services.
- Perform detailed assignments, work under pressure, and relate positively to patients, staff, and visitors.
- Monitor resident care to observe patient condition and monitor delivery of care.
- Participate in the establishment of the care plan and initiate emergency measures as necessary.
- Report changes in patient condition to Physician, family, and other care givers.
- Maintain complete and accurate patient records, providing timely, accurate, and current documentation.
- Maintain a safe, clean, and comfortable environment for patients.
- Update own clinical skills and learn new skills as necessary to perform unfamiliar assignments.
- Adjust daily Nursing Department staffing in both the current shift and upcoming shifts by calling in or dropping personnel as needed.
- Assure timely acquisition of patient observations made by other staff members.
- Assure adherence to new and established policies and procedures by instruction, supervision, and example.
- Collaborate with the DNS in formal performance evaluations.
- Able and willing to work beyond normal working hours and in other positions, temporarily, if necessary.
- Current valid Oregon License as a Licensed Practical Nurse.
- Current CPR card.
- Able to ambulate, stoop, bend, squat, and move a maximum of 60 pounds during routine care for residents for an 8-hour shift.
- Visual and Auditory activity within normal limits with or without prosthetics.
- Frequent walking and transport of residents. Push and pull wheelchairs and other equipment.
- Must function independently, have flexibility, and the ability to work effectively with residents, personnel, and support agencies, and to maintain a calm and harmonious work environment.
