Job Summary:

The Inpatient Case Manager - LVN Position is responsible for assessing, planning, implementing, and evaluating patient care plans to ensure the highest level of care is provided during an acute care episode. This role requires collaboration with physicians, healthcare practitioners, and other stakeholders to develop and implement effective care plans.

Key Responsibilities:

•⁠ Assess patients' needs and develop individualized care plans
•⁠ Collaborate with healthcare teams to implement and evaluate care plans
•⁠ Communicate effectively with patients, families, and healthcare providers
•⁠ Document patient information and care plans accurately and thoroughly

Requirements:

•⁠ Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program
•⁠ Current California Licenses
•⁠ Knowledge of Healthcare and Managed Care preferred
•⁠ Typing 40 words per minute with accuracy
•⁠ Knowledge of computers, faxes, printers, and all other equipment
•⁠ Knowledgeable in MS Office programs (i.e., Word, Excel, Outlook, Access, and Power Point)
