Job Summary:

As a Case Manager at AxessPointe Community Health Centers, you will play a vital role in providing effective community-based coordination and assistance to individuals in need. Your primary responsibility will be to provide specific, measurable, and individualized services to severely mentally disabled or seriously emotionally disturbed clients, their families, and other individuals.

Key Responsibilities:

• Provide ongoing assessment of needs and assistance in achieving personal independence in managing basic needs.
• Facilitate further development of daily living skills.
• Coordinate the Individual Service Plan.
• Monitor and assist in crisis management and stabilization.
• Provide mental health interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment.
• Conduct activities that increase the individual's capacity to positively impact their own environment.

Requirements:

• Bachelor's degree in a mental health-related field and a license in social work preferred.
• Prior community support experience preferred.
• Ability to establish and maintain effective relationships with clients and significant others.
• Valid driver's license, reliable transportation, and adequate levels of insurance and insurability.
• Must be able to meet agency requirements for criminal background check.
