Overview:
Elderwood is seeking a dedicated and experienced Social Worker to take on the role of Chief of Social Services Operations. This leadership position is pivotal in guiding our team of committed social workers.
This role is eligible for a $2500 sign-on bonus.
Our facility, with a capacity of 172 beds, specializes in delivering tailored care in both subacute and long-term settings. The ideal candidate will exemplify our company’s iCARE values, which stand for Integrity, Collaboration, Accountability, Respect, and Excellence. This position plays a vital role in promoting the health and welfare of our residents.
Key Responsibilities:The Chief of Social Services Operations will engage with residents in the nursing home to assess their psychosocial, mental, and emotional requirements. This role involves developing and facilitating access to necessary services while supervising fellow social workers to achieve these objectives. Compliance with state and federal regulations, as well as adherence to company policies, is essential.
The successful candidate will:
- Oversee the admission process for new residents as outlined in the Social Services Department Manual.
- Act as the Unit Social Worker for designated residents or delegate this responsibility based on staffing patterns.
- Function as the Discharge Planner for the facility, completing necessary documentation upon resident discharge.
- Coordinate the PASARR Level II Annual Review for residents.
- Implement and supervise the policies and procedures of the Social Services Department regarding admissions, social work, transfers, and discharges.
- Contribute to the development of comprehensive, interdisciplinary healthcare and discharge plans for residents.
- Participate in Resident Care Planning and Resident/Family Care Plan Review meetings, ensuring communication with other disciplines.
- Provide support and guidance to team members within the Social Work department.
- Supervise and assess the performance of social services staff.
- Collaborate with the Social Work Consultant on matters related to social services interventions.
- Ensure all required documentation for the Social Services Department is completed.
- Clarify the objectives and purpose of the Social Services Department to the professional staff.
- Educate new staff on the Bill of Rights for Residents and Resident Abuse Reporting during orientation sessions.
- Facilitate cooperation with the Ombudsman.
- Baccalaureate Degree in social work or a related field from an accredited institution.
- A minimum of 3 years of experience as a social worker or case worker in long-term care is required.
- Preferred experience includes casework with the elderly or supervisory roles within a long-term care or skilled nursing facility.
- Strong leadership and time management skills are essential.
- A valid Driver's License and a clean driving record are required.
- Ability to engage with the public effectively, manage issues diplomatically, and collaborate with colleagues is necessary.
- Evidence of professional development, continuing education, and community involvement is valued.
- This position necessitates regular interaction with residents, coworkers, visitors, and supervisors, requiring proficiency in reading, writing, speaking, and understanding the English language at an intermediate or advanced level.
