Job Responsibilities:

Position Overview:

The Youth Support Specialist role within the Comprehensive Community Services (CCS) framework is dedicated to assisting youth and young adults in achieving their recovery objectives through tailored skill development and enhancement services.

This position addresses the diverse recovery stages of youth facing mental health, substance use, and co-occurring challenges by leveraging their inherent strengths to fulfill their specified needs.

The responsibilities of the Support Specialist may vary across teams, with services delivered in community settings to align with the needs outlined in the individualized Plan of Care.


Key Responsibilities:

Collaborate as a vital member of the CCS team to provide essential services to youth and young adults dealing with mental health, substance use, and co-occurring challenges.

Focus on the individual's strengths to address their unique recovery requirements. All services will be centered around the individual’s preferences and guided by their input.

Deliver personalized skill development and enhancement services that encompass training in communication, interpersonal relations, problem-solving, decision-making, self-regulation, conflict resolution, and other specific needs as identified in the Plan of Care.

Services will also include training in daily living skills related to personal hygiene, routine tasks, transportation, educational engagement, and connecting with community resources and services (including healthcare) as well as other specific daily living skills identified in the Plan of Care.

Employ a variety of methods to support individuals, including role modeling, monitoring, mentoring, supervision, assistance, and cuing. Provide services in community settings, including residences and other locations.

Engage as a team member in meetings with individuals to develop a Plan of Care, establish and prioritize goals, and track progress toward meeting identified needs.

Ensure thorough documentation of all interactions with individuals and team members to comply with program standards.

Accountable for precise documentation and maintaining a case file in accordance with agency policies, governmental regulations, and applicable local, state, and federal laws.

Participate in staff meetings, training sessions, seminars, and conferences as required. Maintain flexibility in daily scheduling to effectively meet program demands. Uphold agency policies and procedures while fulfilling daily responsibilities.

Additional Duties:
Facilitate transportation for individuals to all CCS-related appointments. Perform other job-related tasks as necessary to fulfill the responsibilities of the position.

Qualifications:

Minimum Requirements:

A High School Diploma or equivalent is required, with a preference for 2 years of experience working with individuals facing mental health, substance use, and co-occurring challenges; a Bachelor's Degree is preferred.

Fulfill all requirements for criminal and caregiver background checks, driving record, and drug screening as mandated by WCS and contractors. A valid driver's license, access to a vehicle, and adequate insurance to meet agency standards are required.

Other Information:

Reporting Structure: This position reports directly to the Director of New and Emerging Initiatives within the Specialized Services Division.

Acts as a liaison with CCS team members, educational institutions, community healthcare providers, family members, and others as necessary.

Knowledge, Skills, and Abilities:
Possess knowledge and skills to work effectively with a population recovering from mental health and substance use issues; demonstrate organizational skills; experience in engaging with youth and young adults in recovery with mental health and/or substance use needs; ability to establish rapport quickly; strong assessment and interviewing skills; comprehensive understanding of mental health needs; solid grasp of substance use issues; capability to navigate challenging situations; flexibility and multitasking ability throughout the workday; effective utilization of time and resources to maintain high-quality CCS services.

Working Conditions:
Work primarily occurs in community settings, including individuals' homes and various community-based locations. Ability to function in an office environment is also required. Average working hours are 40 hours per week.

Physical Demands:

The position requires mobility, with significant time spent in the community, including transporting youth and young adults, conducting home visits, and attending meetings; driving throughout the designated service area; must be capable of navigating stairs.
